Request Portal Access - Sign up here or call your Health Center to make an appointment and learn how to set up your Patient Portal account. 

Log In  - Once you have a Patient Portal account, you will be able to:

• Schedule an appointment
• Exchange messages with your care team
• Review test results and personal health information
• Request prescription refills
• Update your profile and contact information
• Read Health Center news and updates
